Immunization supply chains in low- and middle‑income countries face a critical inflection point. Despite significant past investments, fragmented systems, misaligned incentives, and weak data architecture and use continue to undermine vaccine supply chains and thereby vaccine availability and coverage. This commentary argues that unlocking value trapped in immunization program supply chains requires a deliberate shift from rhetoric to action, underpinned by integrated data systems, shared standards, and strong global and regional leadership and advocacy. Drawing on recent changes in program strategies impacting immunization supply chains, we outline a path forward centered on data‑driven and transparent decision‑making, collaborative partnerships, and the transition to country ownership without sacrificing technical rigor.
